Form image (side layout)
The form image places a picture alongside your form in a two-panel layout, great for product shots, event artwork, or brand imagery. On desktop the image sits next to the form; on mobile it stacks above it.
Form image is in Beta. It requires the beta opt-in and a plan that includes custom branding.

Enabling the form image
The Form Image card only appears in the Design tab once your workspace has opted in. Enable it under Settings → Early Access → Form Images.
Adding an image
- Open a form and go to the Design tab.
- In the Form Image card, click Add image.
- Pick a PNG, JPEG, or WebP file.
- The Crop image modal opens. Reposition by dragging and use the Zoom slider (1×–3×). The crop is fixed to a 4:5 portrait ratio.
- Click Save image. The cropped image is uploaded (max width 1,200px).
Layout controls
Once an image is set:
- Position: place the image panel on the Left (default) or Right of the form on desktop.
- Panel width: how much horizontal space the image takes on desktop, from 25% to 55% (default 40%).
- Replace: swap the image (reopens the crop flow).
- Remove: delete the image and return to the standard centered layout.
How it renders
- On desktop, the image and form sit side by side; the image fills the full height of its panel (
object-cover). - On mobile, the image stacks on top and the form flows below.
- With no form image set, the form uses its normal centered single-column layout.
Plan & availability notes
- The form image lives with the other branding controls, so it requires a plan that includes custom branding. In the pricing table it appears as Form image / side layout (Beta) on the Pro and Business plans.
- It also requires the Early Access opt-in regardless of plan.
